The Lifestyle Management Program Includes:

We have a team of people to help you stay healthy. You’ll work with Flo, a friendly text program, the Connected Care Nurse, and your Clinic Provider.

Here’s what you’ll do:

Learn online about staying healthy and set goals for yourself.

Talk to your patient partner on video or phone every 2 weeks to stay on track.
Visit the Connected Care Nurse every few months to check in at Lackey Clinic.  
Flo will send helpful text messages with reminders.
