Services NOT Available
Chronic Pain Management | Pre-natal Care | Substance Abuse Treatment | Treatment for Severe Mental Illness | Disability physicals and Workmen’s Compensation physicals
The following medications are NOT provided: Controlled substances (including but not limited to narcotics and sleeping pills) or birth control
- New Patients: We are not having walk-in screenings, but we are still accepting online applications, which you can fill out above or you can contact us by calling Amber or Kristyn in the Eligibility Department at 757-886-0608 ext. 253.
- Current Patients: Please do not let your card expire. If your card is going to expire in the next 30 days, you must call Amber or Kristyn in the Eligibility Department at 757-886-0608 ext. 253.
It is the patient’s responsibility to have their Lackey Clinic Card renewed by the expiration date.
We are committed to make a personal primary care provider available to you who provides for your healthcare needs and helps to coordinate your care across all settings, including the hospital, clinics, testing facilities, and other places where you receive healthcare.
